By signing the form, I authorize the use of the first name and last initial of the contest entrant in appropriate program promotional efforts including, but not limited to, website, bookmarks, fliers, postcards, billboards, social media, publications, brochures, and mailings.

By accepting the prizes, the winning regional entries become the sole property of the Interlake-Eastern Regional Health Authority and may be used in promotional efforts including, but not limited to, website, bookmarks, fliers, postcards, billboards, social media, publications, brochures and mailings.

The poster should be original, hand drawn artwork appropriate to the contest theme. To be considered for regional judging, the poster must be received at the Stonewall Community Health Office by March 11, 2022 at 4:00 pm.
